That Penrith trainer Pete Green wasn’t even on the field when he asked the touch judge to stop play to treat injured hooker Mitch Kenny, despite the NRL rules stating the trainer must carry out an “initial assessment” before asking for the game to be brought to a halt due to “serious injury”. This is actually the same trainer's second offence, did the same against the Sharks this year.

That was a penalty for sure, maybe even a professional foul.

I think play should only be stopped when its heading in the direction of the injured player. So in defence that would only be where they are in or behind the defensive line. Obviously there are going to be exceptions to this, but only at the discretion of the referee.
